Counseling Consent Form

I, the undersigned grant consent to receive Biblical/Christian counseling  with Mark Swift. I understand the counseling sessions will be 50-60 minutes and will be confidential. All information will be kept confidential unless legal requirement for the safety and protection of client is expressed and required by law, such occurrences include reports of physical abuse, sexual abuse, rape, threats of suicide or homicide.
